It seemingly happens overnight. Your 13-year-old goes to bed with relatively clear skin and wakes up a few weeks later covered in severe, inflamed, cystic bumps.

Parental panic sets in, followed closely by a frantic trip to the drugstore to buy the strongest “maximum strength” spot treatments available.

Here are the 5 immediate, dermatologist-approved steps to manage a sudden teenage breakout crisis without permanently damaging their skin barrier.

1. Do Not Buy the 10% Benzoyl Peroxide

When confronted with a massive breakout, parents naturally reach for the tube explicitly labeled “Maximum Strength 10%.”

However, dermatological studies have conclusively proven that 2.5% Benzoyl Peroxide is exactly as effective at killing acne bacteria as 10%, but with infinitely less peeling, burning, and severe irritation. Exposing panicked, inflamed skin to 10% BP will only result in a compromised barrier and worsening acne. Find a gentle, low-percentage spot treatment.

2. Ice the Deep Cysts

If your teen develops deep, painful, under-the-skin cystic bumps (the kind that never come to a white head), do not allow them to squeeze them. Squeezing a cyst ruptures the follicle underneath the skin, spreading massive infection.

Instead, wrap an ice cube in a clean paper towel and have them hold it directly against the cyst for three to five minutes. This rapidly restricts blood flow to the area, drastically reducing the swelling, redness, and pain within minutes.

3. Hydrate Aggressively

The immediate reaction to a severe breakout is to dry it out. Teens will often wash their face three times a day and skip moisturizer entirely in an attempt to “starve” the acne.

Dehydrated skin is stressed skin. Stressed skin produces massive spikes in sebum (oil) to compensate, leading to a horrible cycle of dry, tight, oily skin that breaks out endlessly. Even in the middle of an acne crisis, they must apply a lightweight, oil-free gel moisturizer twice a day.

4. Utilize Hydrocolloid Pimple Patches

These are perhaps the greatest modern invention for teen acne.

Hydrocolloid patches serve two incredible purposes:

  1. They physically absorb the pus and debris out of a whitehead overnight.
  2. Even more importantly, they create a physical barrier that prevents the teenager from compulsively picking at their face.

Buy a large pack and have your teen apply them to any active spots immediately after cleansing and drying their face at night.

5. Know When to See a Professional

Over-the-counter interventions (like Salicylic Acid washes and low-dose Benzoyl Peroxide) are highly effective for mild to moderate congestion. However, if your teen’s skin is comprised of deep, painful, scarring cysts that do not respond after 6 weeks of consistent basic care, it is time to call a professional.

Cystic acne can leave permanent physical “ice pick” scars. A dermatologist can prescribe potent topical retinoids or oral medications that can halt the progression of severe, scarring acne before permanent damage sets in.
